🌐 Summary
Diabetes annual reviews are needed to assess how well diabetic patients are managing their condition, and to identify any complications or increased cardiovascular risk. Diabetic annual reviews are completed in 2 parts within the PCN:
- ‘Part 1’ is held by an HCA who will complete all 8 care processes with the patient. They’ll also book the ‘Part 2’ appointment 1-2 weeks ahead, to allow time for the patients’ results to come back.
- ‘Part 2’ for patients with T2DM is held by a PN, ANP or DSN who will complete a holistic review of the patient’s condition and test results, addressing any changes needed to their lifestyle of medication regime.
The patient is then discharged back to their GP for follow-up, with an interim HbA1c review after 3-6 months (dependent on HbA1c result). For more information, see the document below.

Book into slot ‘HCA Diabetes Part 1 Review EA’. If not available, book into ‘NLPCN Diabetes or Hypertensive Review Only’.
✅ What this service does
- Blood tests for HbA1c, renal function and lipids
- Urine ACR
- Blood pressure
- Diabetic foot check
- Height, weight and calculated BMI
- Record smoking status
- Onward referrals (e.g., for foot health, smoking cessation or weight management)
- Follow-up appointment to review results
❌ What this services does not do
- Action test results which require urgent attention, as all results are sent directly to individual practices. GPs are required to follow-up on any results deemed to need urgent attention if the results are viewed before the patient’s ‘Part 2’ appointment in the PCN.
- Care planning on ‘Part 2’ reviews for patients with T1D.
